Our History

We have been providing solutions for seniors and caregivers in our area since 2000

Community Partners Two Harbors Living at Home Block Nurse Program was established by community professionals and volunteers in March of 2000 to assist older adults of Two Harbors to remain in their homes safely for as long as possible.

A local study conducted around the time Community Partners was founded indicated that there was a demonstrated need for the creation of flexible volunteer services to help an aging population live at home. Community Partners was created to help elders with service coordination and volunteer services; and to support families, friends, and neighbors in caring for loved ones.
